Athletics Forms
Please read the instructions below, then click here to sign in to RankOne.
If you already have a RankOne account:
Log in or select Forgot Password to reset your sign-in. You should automatically be linked to your account from last year, but the forms for the current year will be listed.
Your student will initially be shown Out of Compliance. Click on gray View button to see the two forms that need signatures: TAPPS Student Profile and TAPPS Signature Page.
After completing the online forms, you will need to submit the TAPPS Physical Form and Medical History. Download and print the form from the box on the right.
Take this form to your student's physician for completion and signature. Physicals are good for one calendar year from the date of the exam.
Once the form is completed by your physician, you will need to turn the paper form in to the Athletics Office or the Trainer.
If you have more than one student at JPIIHS, you will need to click on the Manage your Students or Add a Student Athlete button to switch between students and repeat the above process for each child.
If you do not have a RankOne account:
Please click on the link above and create a Parent Portal for yourself. Only one parent is required to complete this process.
Using your student's ID number or Date of Birth, look up your student to be automatically linked to the student's RankOne page.

Transfer Students
All transfer students in grades 10-12 must complete the Student Transfer Form (STF) and submit. This form can be found on your Rank One dashboard.
Transfer students must also fill out the top portion of the Previous Athletic Participation Form (PAPF). Print and submit the PAPF to the JPIIHS Athletic office to complete.
